Supportlinks in a nutshell

Needs assessment and service coordination provider Supportlinks is the gateway
to piublicly funded long term disability support for individuals living in the
MedCentral DHB region.

Assessment facilitators and service coordinators work with people to identify
disability related needs and support options for people with life long and/or
age related disability.

MidCentral DHB manages funding of this support for those over the age of 65 in
this area. For under 65s the funding is managed directly by the Ministry of
Health.
